Abstract:  While the alternate-site infusion practice setting offers many unique pharmaceutical challenges, the amount of documentation required to comply with legal, regulatory and accrediting organizations can be overwhelming. In this article, the author discusses professional pharmacy and accrediting organizations, American Society of Health-System Pharmacists “Technical Assistance Bulletin” and United States Pharmacopeia <1206>. Tables provide information about risk-level classifications established by both these groups. In addition, the author provides a sample policy and procedure for compounding records and a total parenteral nutrition compounding record. In summary, the compounding record is an important tool to ensure the quality and integrity of the sterile products prepared in the alternate-site/home-infusion pharmacy.
