Abstract:  A pharmacy will be more organized, efficient and profitable if it uses a well-organized chart system to catalogue essential business information. The samples shown in this article are similar to the charting system used at Custom Prescriptions and Consulting Services in Fort Wright, KY (with which the authors are affiliated). The authors encourage readers to adapt the forms featured and invite feedback as to formats. Sample forms shown are for acute-care prescriptions, hormone symptoms, disease-state management indicators, patient information, prescriptions, pharmacist’s interventions and notes to file and SOAP notes.
