Abstract:  The author recounts a personal experience involving a patient hospitalized as a result of an error made in a preparation compounded by his/her staff and goes on to discuss the duties of a compounding pharmacist. These include using only the highest grade of chemicals, being sure that staff are competent and well trained and doing no harm to the public. The author concludes that the most critical lesson is that compounding pharmacists have the potential to ruin patients’ lives and change them irreparably forever. He/she admonishes readers to be certain they understand this before compounding a preparation for a patient, because the legal system will certainly hold them responsible.
