Abstract:  The author, affiliated with Clarian Health Partners, Inc., describes the 1997 merger of three Indianapolis hospitals, generalizing to provide information about the merger process and how such consolidations affect the pharmacy department and compounding practices. She identifies the three phases of a merger as being the review phase, when all aspects of the compounding service are evaluated by the compounding pharmacist and research is done to justify revisions; the planning phase, when formulation changes are discussed, policies are written for compounding, and a committee is formed; and the activation phase, when compounding services are offered throughout the new healthcare system. A list of new pharmacy services for Clarian Health Partners, Inc., is provided, along with a description of other pharmacy activities (batch compounding, education, unit-dose packaging of tablets and capsules and provision of information and assistance to retail pharmacies).
