Abstract: This article is intended to address the activities of the caregiver and encourage the need to address adverse events or medical errors. The advantage of disclosing adverse events or medication errors is to avoid further potential errors and determine/correct the cause of the errors. There should be awareness between patients and caregivers on the proper use and administration of intended compounded sterile preparations. Patients/patient’s relatives should ask questions about their medications and be knowledgeable or cognizant on the appearance of the preparation or specified dosage of the medication. Caregivers should be open and explicit about the medications being administered or prescribed by the doctor and the rationale for such medications. This module will address these sections in a practical manner and provide practice clarification to the standards.
