Abstract:  This chart is reprinted from the 1999-2000 Survey of Pharmacy Law by the National Association of Boards of Pharmacy. It provides information for each state as to the number of pharmacy license categories; whether nonresident pharmacies must be licensed/registered; registration fee; initial licensure/renewal fee; renewal schedule; initial controlled substance fee; controlled substance renewal fee; controlled substance renewal schedule; and whether the board’s state act provides for separate institutional pharmacy regulations, separate nuclear regulations pharmacy and separate home-care pharmacy regulations.
